Eligibility requirements:

Legal residents of Delaware
U.S. citizens or eligible non-citizens
Full-time students enrolled in a program preparing them for licensure as an optometrist in Delaware.
Award:
Loan not to exceed cost of tuition, fees, and other direct educational expenses
Renewable for three additional years
Loan forgiveness provisions at a rate of one year of employment as an optometrist in Delaware for one year of loan
Deadline:
All application materials must be received by 4:30 p.m. on Friday, March 27, 2009.
Your High School Academic record Report and Transcript Request or College Grade Release Form must be submitted directly to the Commission by your high school or college and be received by 4:30 p.m. on Friday, March 27, 2009.

You can apply online beginning December 1, 2008, for the 2009-2010 Optometry Incentive Program by completing the Common Merit Application on DHEC's secure Student Account Access site at https://www.doe.k12.de.us/high-ed/student/login.aspx. First-time users must establish an account and provide a valid email and mailing address before proceeding to the application. Incomplete applications cannot be processed. Please be sure to provide all of the required information:


Compeleted application
Essay of 1,000 to 1,200 words
(a) High School Academic Record Report and Transcript Request (high school seniors and applicants with less than 12 college credits), or (b) College Grade Release Form (applicants with 12 or more college credits)
